Dear Comrades,
                        You have to submit your Life Certificate to the bank/ Post Office through  where you get your monthly pension . The form will be issued by the bank/post office.

You take your Bank Pass Book, original PPO Book, PAN Card and Adhaar card  both with Xerox Copies along with you for reference and bring back and keep with you very carefully.
